FDA Clears Denali's AVLAYAH, the First Brain-Penetrating Enzyme Therapy, Cracking Hunter Syndrome's Neurologic Frontier After Two Decades

Denali's tividenofusp alfa becomes the first FDA-approved biologic engineered to cross the blood-brain barrier, breaking a long rare-disease rejection streak and validating a transport-vehicle platform that could unlock CNS access for many more drugs.

Overview

The U.S. Food and Drug Administration has granted accelerated approval to Denali Therapeutics’ AVLAYAH (tividenofusp alfa-eknm), the first FDA-approved biologic specifically engineered to cross the blood-brain barrier and reach the central nervous system, according to the FDA’s press announcement. The drug is indicated for the neurologic manifestations of Hunter syndrome, a rare and progressive pediatric disease also known as mucopolysaccharidosis type II (MPS II), and is the first therapeutic innovation for the Hunter syndrome community in nearly 20 years, Denali said in its press release.

The approval, announced March 25, 2026, carries implications beyond one ultra-rare disease. Tividenofusp alfa is the first clinical validation of Denali’s proprietary TransportVehicle platform, a protein-engineering approach that hijacks the transferrin receptor to shuttle biologic drugs across the blood-brain barrier. Analysts framed the decision as a signal that the FDA under Commissioner Martin Makary remains willing to accept biomarker-based approvals for rare diseases, reports BioPharma Dive.

What We Know

Hunter syndrome is caused by mutations in the gene encoding iduronate-2-sulfatase (IDS), an enzyme that clears glycosaminoglycans such as heparan sulfate from cells. Without functional IDS, those sugars accumulate, causing progressive damage to organs, bones, and — in the severe neuropathic form — the brain. Hunter syndrome affects approximately 500 patients in the United States, according to BioPharma Dive.

The only existing approved treatment, Takeda’s Elaprase (idursulfase), is a conventional enzyme replacement therapy that cannot cross the blood-brain barrier. It addresses peripheral symptoms but leaves the neurologic decline of the severe form untreated. AVLAYAH fuses the IDS enzyme to a Fc fragment engineered to bind the apical domain of the transferrin receptor, enabling receptor-mediated transcytosis into the brain as well as peripheral tissues, Denali explained in its announcement.

The FDA granted accelerated approval using cerebrospinal fluid heparan sulfate as a surrogate endpoint. In the pivotal Phase 1/2 trial, 44 patients with measurements at week 24 showed an average 91 percent decrease from baseline in CSF heparan sulfate, with individual reductions ranging from 72 percent to 98 percent, per the FDA announcement. Denali reported that 93 percent of treated patients reached CSF heparan sulfate levels within the normal range by week 24, according to its press release.

AVLAYAH is administered as a once-weekly intravenous infusion and is approved for presymptomatic or symptomatic pediatric patients weighing at least five kilograms who have not yet developed advanced neurologic impairment, the FDA said. Continued approval will depend on verification of clinical benefit in a confirmatory trial — the global Phase 2/3 COMPASS study, which Denali says compares AVLAYAH to idursulfase and is currently enrolling its second cohort, according to the company’s press release. The drug carries breakthrough therapy, fast track, priority review, and orphan drug designations, the FDA noted.

Alongside the approval, Denali received a rare pediatric disease priority review voucher — a tradable asset valued at more than $150 million on the secondary market, BioPharma Dive reported.

Pricing and Access

AVLAYAH will be priced at a premium to existing therapy. Annual maintenance cost runs to roughly $811,000 for a 30-kilogram patient, compared with approximately $500,000 per year for Takeda’s Elaprase, according to BioPharma Dive. Dosing scales with body weight, as is typical for enzyme replacement therapies.

The drug’s label lists infusion-associated reactions, upper respiratory and ear infections, fever, anemia, cough, vomiting, and rash among the most common adverse effects. Hypersensitivity reactions including anaphylaxis, infusion-associated reactions, anemia, and one case of membranous nephropathy — a kidney disorder — have also been reported, Denali disclosed. AVLAYAH is not recommended for use in combination with other enzyme replacement therapies.

Regulatory Context

The decision lands in a charged moment for rare-disease drug development at the FDA. Earlier in the review cycle, the agency rejected Regenxbio’s gene therapy for Hunter syndrome, raising concern in the investor community that regulators had grown skeptical of biomarker-based approvals, BioPharma Dive noted. The Avlayah decision reverses that narrative for at least one modality.

Stifel analyst Paul Matteis described the clearance as a “clear step in the right direction” for rare-disease sponsors and said the FDA’s willingness to accept heparan sulfate as a surrogate “draws a line in the sand” on the agency’s biomarker flexibility, per BioPharma Dive.

What We Don’t Know

The approval is accelerated, not full. Whether the drug produces measurable clinical benefit on cognitive, developmental, or functional endpoints will not be settled until the COMPASS confirmatory trial reports out. Biomarker correction — even steep, durable CSF heparan sulfate reduction — has not always translated into preserved cognition in prior Hunter syndrome programs, and long-term safety data in pediatric patients will continue to accumulate.

The broader question is whether the TransportVehicle platform will succeed against other neurologic conditions. Denali has applied the same transferrin-receptor approach to investigational programs for Alzheimer’s disease and other lysosomal storage disorders, but none of those have yet demonstrated clinical efficacy. AVLAYAH’s approval proves that the platform can move a fused enzyme into the CNS in patients; it does not yet prove that the strategy will work for antibodies, oligonucleotides, or other cargo types Denali and competitors are pursuing.

Payer response is also unresolved. At north of $800,000 per year and with a lifelong dosing schedule, insurers will need to negotiate access for a small but expensive patient population, and state Medicaid programs — which cover many pediatric rare-disease patients — have so far given no public guidance on coverage for AVLAYAH.
