Dana-Farber's CAR-PRISM Trial Achieves 100% MRD-Negativity in High-Risk Smoldering Myeloma, Pushing CAR-T Into Pre-Cancer Territory
A single infusion of Johnson & Johnson's Carvykti rendered all 20 patients in a Dana-Farber phase 2 trial undetectable for residual disease, the first time CAR-T therapy has been tested in a precursor cancer setting.
Overview
A Dana-Farber Cancer Institute phase 2 trial has reported that every patient treated with Johnson & Johnson’s Carvykti achieved undetectable disease within two months, according to results published in Nature Medicine on April 20, 2026 and presented at the American Association for Cancer Research Annual Meeting in San Diego. The study, called CAR-PRISM, is the first to test CAR T-cell therapy in patients with smoldering multiple myeloma, a precursor condition that can precede active cancer by years.
What We Know
The trial enrolled 20 patients with high-risk smoldering multiple myeloma and gave each a single infusion of ciltacabtagene autoleucel, the BCMA-directed CAR-T therapy marketed as Carvykti. Patients received no induction or bridging therapy before the infusion, a deliberately stripped-down design meant to isolate the effect of the cell therapy itself, as reported in the Nature Medicine paper.
All 20 patients reached minimal residual disease negativity at a sensitivity of 10⁻⁶ within a median of one month, and all remained negative at a median follow-up of 15.3 months, per the Nature Medicine paper. No patient progressed to active multiple myeloma during follow-up, and no deaths were recorded. Six patients have now been followed for more than 18 months without relapse.
On safety, the Nature Medicine report documented cytokine release syndrome in all 20 patients but capped entirely at grade 1 or 2, with no grade 3 or higher events. Transient grade 3 or 4 neutropenia was the most common adverse event. Seven patients developed non-ICANS neurologic toxicities: four cases of facial nerve palsy that fully resolved, and three with mild residual motor symptoms that improved over time.
The context matters. Carvykti is currently approved by the Food and Drug Administration for relapsed or refractory multiple myeloma after prior lines of therapy, based on data from the CARTITUDE-1 trial that showed an overall response rate of 97.9 percent in heavily pretreated patients. Using it in a precursor state, before patients develop active cancer, inverts the traditional escalation model in hematologic oncology.
In its coverage of the AACR presentation, STAT noted that the only therapy currently approved for high-risk smoldering myeloma is Darzalex, an antibody treatment that patients typically take for years without reaching the deep molecular responses seen in the CAR-PRISM cohort. STAT also reported that roughly 20 percent of patients on the current standard progress within five years.
What We Don’t Know
The trial is small, single-arm, and conducted at a single institution. The Nature Medicine paper itself lists no control group, meaning the 100 percent MRD-negativity rate cannot be directly compared to a matched group of patients on observation or Darzalex. Whether the same results will hold in a larger, multi-center, randomized trial is an open question.
Durability is the other unknown. The median follow-up of 15.3 months is meaningful but short relative to the natural history of smoldering myeloma, which can remain indolent for years before transforming. Whether MRD-negativity at this time point will translate into long-term freedom from active cancer, or whether some patients will relapse at three or five years, will require additional follow-up.
Cost and access are not addressed in the published data. Carvykti is a personalized cell therapy that requires apheresis, manufacturing, and inpatient administration; using it at the pre-cancer stage raises questions about selection criteria and healthcare economics that the 20-patient study was not designed to answer. STAT noted that the results have prompted researchers to consider whether CAR-T therapy could shift the treatment paradigm from disease management toward potential cure, but the article flagged that this remains a hypothesis rather than an established outcome.
The neurologic side effects also warrant attention. Four cases of facial nerve palsy in a 20-patient trial, even if all resolved, is a signal that will need to be tracked carefully as the therapy is evaluated in larger populations of patients who, by definition, do not yet have cancer.
Analysis
The CAR-PRISM findings mark a conceptual shift more than an incremental one. CAR-T cell therapy was developed as a last-line option for patients whose cancers had already resisted multiple prior treatments. Taking the same therapy and deploying it against a precursor condition, as the Nature Medicine authors describe, is an attempt to intercept disease before the tumor burden, immune dysfunction, and accumulated mutations of active multiple myeloma make treatment harder.
The safety profile supports the hypothesis. Patients with smoldering disease have healthier immune systems and smaller tumor loads than those in Carvykti’s existing label, which may explain why cytokine release syndrome was uniformly low-grade in the trial. But the neurologic toxicities in seven of 20 patients underscore that a one-time infusion in a patient who feels well is not risk-free, and regulators will likely demand robust safety data before any label expansion.
For Johnson & Johnson, which funded the study, CAR-PRISM is a strategic read on whether Carvykti’s commercial life can extend into earlier lines of care. For patients, the question is whether “cure” is finally a reasonable word to use about multiple myeloma, or whether the 15-month follow-up is simply too short to tell.
